If the incident requires urgent action, ensure safety measures are in place and emergency services are contacted first and complete this at your next available opportunity.

For non-emergency incidents, please contact your Regional Manager or State Manager as soon as possible.

Submit Incident Report

For all incidents, you must complete the online incident form above. In your description, include details of what was happening before, during, and after the incident. Answer the relevant questions and lodge the incident as soon as you can. All incidents reported will be reviewed by our admin team, and someone will follow up promptly when required.
